Anyone living in the practice area (within a 10 mile radius of the practice) is most welcome to register with our practice. To do so you should bring a form of photographic ID and proof of address in the form of a utility bill or bank statement.

You will then be asked to book an appointment for a health check within one month of registering. This health check gives us an opportunity to meet you and to learn of your past medical history (your medical records may take a few weeks to arrive)

Medical treatment is available from the date of registration. Please contact reception for further information.

Changes to Personal Details

If you change your address etc. please notify reception staff as soon as possible and provide a proof of address in the form of a utility bill or bank statement. This is so that our records are kept up to date. Contact Telephone Numbers would also help in the event that we need to contact you.
